Appearance/layout

The *layout setting lets you create different layouts for any page or any set of pages. So the sidebar could be on the left, or there could be no sidebar, the links at the top of the page could appear elsewhere, etc.
Features we'd still like to have
- √Ability to suppress table of contents on all Pattern cards - http://wagn.org/wagn/apply_cardtype_plus_toc_to_cards_of_that_type
- √Navigation bar we can edit across the top (this is possible now, it's up to us to implement it. --John)
- Auto-create pattern cards that are linked to from "+related patterns" (or is uncreated related patterns enough?)
